Registration

Please fill out the registration form.

Then email the completed form to mnfoottechinfo@yahoo.com, or print it and mail to MN Foot Tech, P.O. Box 511, Stillwater, MN 55082.

Payment can be mailed along with the registration form, or pay on the first day of camp.

If you'd prefer to pay online with PayPal, choose the week under the specific camp you want to attend, then click "Add to Cart". You may select more than one camp before checking out. Please note that a $5.00 processing fee is added when using PayPal.
